Why were some results within this results set not signed with the rest of the set?

I have a sample set with 16 total injections performed.  Those 16 injections were batch processed to create a single results set with 16 results.  Level 1 and level 2 signoffs were applied to that results set using an appropriate report method.  However, only 11 of the 16 injections were signed level 1.  The remaining 5 injections did not receive any signatures, yet they were locked as if they had received both levels of signatures.  How did a portion of this results set go unsigned?

  • Check the sample set for those unsigned injections. In the Processing column was it set to Don't Process or Report for those injections? If so no results would be generated for them.
    Also check your report method used to sign off the results. Is there a filter or something which excluded those results and therefore they weren't signed off?
    Check that the number of channels in sample set matches number of results..view channels as results the number should be the same.
  • All injections were set to Normal in the Processing column.  Results are displayed in the report for all 16 injections, although the sign off table displayed at the end of the report confirms only 11 of 16 results were signed.  This sample set has 16 channels and 16 results.
  • Is your tablespace sufficient in the project to execute all these signoffs? Check remaining tablespace to be sure.
    Possibly the report method may be corrupt as well. Try a different one to compare. 
  • We had this problem also once.
    Our problem was the high processing time and people who did not wait on all results to be processed.

    They opened the result set in review before it was processed completely and did not notice the missing injections. That might also be the case...

    Check the time of sign off 1 and result creation.

    You can also set in your system policies the "enforce sign off 1 before sign off 2" policy.
    So sign off 2 prompts an error for some results and reviewer will be notified in advance.
